Adult Care Management Services to guide families, individuals of 50 years and older, with incapacitated disability, physically or mentally
Professional consultation/counseling to enhance the quality of life for this population
Prioritizing needs, developing customized care plans, setting up advocacy, and referral to resources and making sure the plan is implemented and followed so as to have a positive outcome by offering a wide spectrum of services individually tailored to the needs of our clients
We also offer Power of Attorney services for the elderly with no family or advocate
On-site Extensive Assessment of the client’s circumstances, including, but not limited to, personality profile, family dynamics, and financial resources available for his/her care and safety.
Direct Consultation and Coordinating of daily living needs services with providers or therapist;
Arranging and Coordinating Medical Appointments/Treatments with the authority of the family, guardians or legal representative;
Doctor’s visit companionship, and follow up
Bill Payment Assistance, and preparation of monthly budgets
Assistance with Groceries, cooking, and pet care
Continuous Updates on client’s care and progress with the family or representative;
Arranging housekeeping services
Research on assisted living facilities as the need arises; and finding a perfect matched with Clint’s preference and financial ability
Assistance with the Application for benefits or resources available in the community to pay for the services; otherwise services be offered self-pay
